Structured log for recording incidents of harassment, stalking, or abuse, formatted for use in police reports, civil injunction applications, and employment grievances.
When to use this template
Start this log as soon as you experience behaviour that concerns you, before it escalates. Courts, police, and employment tribunals all place considerable weight on contemporaneous records. Each entry should be completed as soon as possible after the incident while your memory is fresh.
When NOT to use this template
This is an evidence-gathering tool, not a letter to be sent. Do not send this log to the person who is harassing you. Keep it securely. If you are in immediate danger, call 999.
Legal Basis
Protection from Harassment Act 1997 (criminal offences and civil tort of harassment); Stalking Protection Act 2019; Online Safety Act 2023. A course of conduct requires at least two incidents. The evidence log is designed to meet the requirements for a civil harassment injunction or police disclosure of evidence.
Common Mistakes to Avoid
- ✗Not recording incidents at the time — retrospective logs carry less weight
- ✗Not keeping copies of all physical evidence (screenshots, letters, messages) with the log
- ✗Not recording witnesses' names even if you do not yet need them
- ✗Destroying or deleting original messages — preserve originals even if upsetting to keep
