Who uses this form
Parties to fast track or multi-track civil claims. Often completed with legal advice as the directions sought significantly affect litigation timetable and costs.
When to use it
Return within the deadline set in the Notice of Proposed Allocation. The court uses your answers to set a case management timetable leading to trial.

Return to the court managing the proceedings. Serve a copy on all other parties at the same time.
