Prevention of Future Deaths Report

A report issued by a coroner where evidence at an inquest reveals a risk of future deaths. The recipient (individual or organisation) must respond within 56 days outlining actions taken or proposed.

Where a coroner is under a duty under paragraph 7(1) of Schedule 5 to the Coroners and Justice Act 2009 to make a report to prevent other deaths, regulation 28 of the Coroners (Investigations) Regulations 2013 requires the report not to be made until the coroner has considered all the documents, evidence and information relevant to the investigation. A copy must be sent to the Chief Coroner and to every interested person the coroner considers should receive it, and the Chief Coroner may publish the report.

Under regulation 29, a person who is under a duty to respond to a Prevention of Future Deaths report must set out details of any action taken or proposed, with a timetable, and the response must be provided to the coroner who made the report within 56 days of the date the report was sent.
