Who uses this form
Parties to civil proceedings who are changing their solicitor, or acting in person where previously represented (or now instructing a solicitor when previously unrepresented).
When to use it
File as soon as the change takes effect. The other parties and the court must be informed promptly to ensure all correspondence goes to the right address.
Where to file
File at the court dealing with the proceedings. Serve on all other parties simultaneously.

Common mistakes to avoid
Delay in filing — the other side may serve documents on the old solicitor, causing you to miss deadlines
Not serving a copy on all other parties at the same time as filing
Not including the new address for service clearly
Filing without the signature of the incoming solicitor or the client (as appropriate)
